Al-Fatiha as ruqyah

بِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ ٱلْحَمْدُ لِلَّهِ رَبِّ ٱلْعَـٰلَمِينَ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ مَـٰلِكِ يَوْمِ ٱلدِّينِ إِيَّاكَ نَعْبُدُ وَإِيَّاكَ نَسْتَعِينُ ٱهْدِنَا ٱلصِّرَٰطَ ٱلْمُسْتَقِيمَ صِرَٰطَ ٱلَّذِينَ أَنْعَمْتَ عَلَيْهِمْ غَيْرِ ٱلْمَغْضُوبِ عَلَيْهِمْ وَلَا ٱلضَّآلِّينَ

In the name of Allāh,^[2] the Entirely Merciful, the Especially Merciful.^[3] [All] praise is [due] to Allāh, Lord^[4] of the worlds - The Entirely Merciful, the Especially Merciful, Sovereign of the Day of Recompense.^[5] It is You we worship and You we ask for help. Guide us to the straight path - The path of those upon whom You have bestowed favor, not of those who have earned [Your] anger or of those who are astray.

Al-Falaq for refuge

بِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ

قُلْ أَعُوذُ بِرَبِّ ٱلْفَلَقِ مِن شَرِّ مَا خَلَقَ وَمِن شَرِّ غَاسِقٍ إِذَا وَقَبَ وَمِن شَرِّ ٱلنَّفَّـٰثَـٰتِ فِى ٱلْعُقَدِ وَمِن شَرِّ حَاسِدٍ إِذَا حَسَدَ

Say, "I seek refuge in the Lord of daybreak From the evil of that which He created And from the evil of darkness when it settles And from the evil of the blowers in knots^[2014] And from the evil of an envier when he envies."

An-Nas for refuge

بِسْمِ ٱللَّهِ ٱلرَّحْمَـٰنِ ٱلرَّحِيمِ

قُلْ أَعُوذُ بِرَبِّ ٱلنَّاسِ مَلِكِ ٱلنَّاسِ إِلَـٰهِ ٱلنَّاسِ مِن شَرِّ ٱلْوَسْوَاسِ ٱلْخَنَّاسِ ٱلَّذِى يُوَسْوِسُ فِى صُدُورِ ٱلنَّاسِ مِنَ ٱلْجِنَّةِ وَٱلنَّاسِ

Say, "I seek refuge in the Lord of mankind, The Sovereign of mankind, The God of mankind, From the evil of the retreating whisperer^[2016] - Who whispers [evil] into the breasts of mankind - From among the jinn and mankind."^[2017]

Asking the Healer

أَذْهِبِ الْبَاسَ رَبَّ النَّاسِ، اشْفِ وَأَنْتَ الشَّافِي، لاَ شِفَاءَ إِلاَّ شِفَاؤُكَ، شِفَاءً لاَ يُغَادِرُ سَقَمًا

Remove the illness, Lord of people. Grant healing, for You are the Healer. There is no healing except Your healing, a healing that leaves no sickness.

Sahih al-Bukhari 5675

In brief: The Prophet ﷺ said this when visiting a patient or when a patient was brought to him.

Refuge from pain

أَعُوذُ بِاللَّهِ وَقُدْرَتِهِ مِنْ شَرِّ مَا أَجِدُ وَأُحَاذِرُ

I take shelter in Allah and His power from the harm I feel and fear.

Place your hand on the painful area; say 'Bismillah' three times, then this supplication seven times.

Sahih Muslim 2202

In brief: After saying Bismillah three times with a hand on the painful place, say this seven times.
